What exactly are your concerns? HIPPA laws are very strict when it comes to patient medical records and their privacy.

My concern is that the authorization they're asking me to sign gives them access to all of the information that clinic has on me, or, if the clinic does not have at least the minimum information they want, to submit that information from another doctor.

Does your employer ask you to give them access to your health information?
